Touch of Elegance Spa – Liability Waiver & Disclaimer

​By scheduling or receiving services from Touch of Elegance Spa, you acknowledge and agree to the following:
 

1. Health & Safety Notice
Touch of Elegance Spa offers holistic wellness services including massage therapy, bodywork, hydrotherapy, and energy work. These services are not a substitute for medical examination, diagnosis, or treatment. If you have a medical condition or are under medical care, please consult your physician before booking any service.
 

2. Voluntary Participation & Assumption of Risk
All services are provided on a voluntary basis. By participating, you affirm that you are in good health and fully understand the potential physical and emotional effects of services such as deep tissue massage, lymphatic drainage, cranial sacral therapy, sauna use, float spa, and other treatments. You assume full responsibility for your participation and waive all claims for any personal injury or condition that may result.
 

3. Contraindications & Disclosures
Certain conditions (e.g., pregnancy, high blood pressure, recent surgery, skin conditions, infections, or chronic illnesses) may require medical clearance or service modifications. It is your responsibility to disclose any relevant health information when booking or prior to your appointment.
 

4. Services for Minors
Clients under the age of 18 must be accompanied by a parent or legal guardian and must provide written consent prior to receiving any services.
 

5. Booking Platform & Consent
Appointments made via the Pocket Suite App constitute your agreement to these terms. By booking a service, you confirm that you have read, understood, and agreed to this liability waiver and disclaimer.
 

6. Release of Liability
To the fullest extent permitted by Utah law, you hereby release, waive, and discharge Touch of Elegance Spa, its owners, employees, and practitioners from any and all claims, liabilities, or damages arising from your participation in any service offered.
